Symptoms
A symptom of prostatitis depends on cause.
• Difficulty urinating for example hesitant or dribbling urination
• Burning sensation or pain when urinating called has dysuria
• Frequent urination, mainly at night (nocturia)

• Blood in urine (haematuria)
• Cloudy urine
• Pain in groins, abdomen and between the scrotum and rectum.
• Painful ejaculation with discomfort or pain in penis or testicles.
• Bacterial infection gives flu like symptoms.
Causes
Prostatitis can be caused by bacteria that leak into the prostate gland from the urinary tract (the most common bacterial cause) and from direct extension or lymphatic spread from the rectum. It can also result from various sexually transmitted organisms such as Neisseria gonorrhoeae, Chlamydia trachomatis, or HIV.
RISK FACTORS
• Recurrent urinary tract infection (UTI)
• Enlarged prostate gland
• Rectal intercourse
• Having functional or structural urinary tract abnormalities
• Trauma to pelvic muscles
• Dehydration
Diagnosis
On bases of history, doctor will do following examination and test
Digital rectal exam– to check prostate gland for pain and swelling may include prostate massage to collect sample of seminal fluid.
Urinalysis – Urine culture to check bacteria and uti
Blood test– A blood test helps to measure prostate specific antigen (PSA), protein made by gland. Increased level in blood test indicates either benign prostrate hyperplasia, prostatitis, or prostate cancer.
Invasive test for prostatitis are
Cystoscopy to view inside bladder and urethra to rule out urinary tract problems but does not diagnose prostatitis.
Transrectal ultrasound is done to rule out prostate gland abnormalities.
Treatment
Treatment depends upon underlying cause
All type of prostatitis need pain management. For bacterial infection, antibiotic are prescribed.
HOMOEOPATHIC MEDICINE
APIS MELLIFICA
Patient has desire to pass urine frequently but that leads to burning pain. Good remedy for acute prostatitis where patient finds difficultly in retaining urine even for few seconds. Pain is stinging and burning in nature. Scanty and dark urine with burning and stinging pain while passing urine even last drop of urine pains a lot. Complaints aggravated by heat and pressure, feels better by cold bathing and open air.
CANTHARIS
Patient complaints of urine come in drops with severe burning sensation along with severe scalding before, during and after passing urine. There is constant desire to pass urine but with burning sensation and urine is passed drop by drop. There is desire for stool while urinating. Patient is mostly in anxious state with urge to pass urine and stool with sexual erethism. Burns like fire in the bladder with bloody urine. Patient suffering from gonorrhea has this violent inflammation with burning and tenesmus of bladder with retention or suppression of urine.
THUJA OCCIDENTALIS
Prostatitis where patient is unable to control urine and has sudden onset and desire to empty the bladder quickly. There is rush and hasten to pass urine. Sometimes involuntary urine may pass in night (nocturnia). Patient has history of gonorrhea and syphilis that has been suppressed or maltreated. Urine is cloudy and foamy or mucus maybe present in high levels. Pain and burning felt near neck area of bladder, with urgent and frequent desire to urinate. Complaints are worst at night, at 3 am and 3 pm, from cold, damp air and better by drawing up a limb.
CHIMAPHILA UMBELLATA
There is retention of urine with severe pain; patient has difficulty to pass urine with burning and scalding during urination. Urine contains ropy mucus and blood in it and is greenish black in colour. The peculiar position of patient standing with feet’s wide apart and stooping in forward direction to pass urine with much straining. Generally condition is better walking and aggravates by sitting. Acute prostatitis with dysuria and retention of urine has a sensation in perineum as if sitting on ball.
NITRIC ACID
There is an urge to empty the bladder is frequent but urine is scanty in quantity and emits a highly offensive odour. Patient complaints of burning in urethra while passing urine. While passing urine feels cold on passing pus discharge from urethra may also be present in some patients. Urine is scanty and highly offensive smelling with thin stream while passing.
SABAL SERRULATA
Complaints are frequent urging to pass urine mostly at night with difficulty in passing urine. The area around prostrate feels cold and ejaculations is painful or impossible. There is prostatic emission with stool. Semen flows slowly and feels thick, but causes hot feeling along the cords and testicles tightly drawn up, painful in nature. There is feeling of coldness of external genitals giving dull aching in prostate extending in abdomen. Complaints aggravates after coitus.
